RESUMO

A progressively increasing percentage of the elderly live during the last years of their lives in nursing homes. Although these institutions are intended to mimic life at home as much as possible, they have characteristics that make them quite similar to a "nosocomiun", i.e. an establishment for the treatment of the sick. The very coexistence among the elderly, the fact of sharing caregivers and the very significant exposure to third parties, together with the frequent predisposing diseases to infection in this population, make infection frequent among residents and also easily transmissible. This leads us to ask what can be done to prevent infection in this environment and more specifically what is the state of the art of the matter in a Western European nation such as ours. The Board of Trustees of the Health Sciences Foundation has asked itself a series of questions on the subject of infection prevention in Nursing Homes, the structure of procedures, the legislation available, compliance with the measures indicated, the best indicators of the processes and therefore, the need to promote in Spain a document of recommendations to avoid infections in this poplation whose morbidity and mortality need not be highlighted. To this end, a multidisciplinary group of experts in different aspects of this problem has been convened and asked the proposed questions. The questions were discussed by the group as a whole and led to a series of conclusions agreed upon by the participants. The results of the meeting are reported below.

RESUMO

There is accumulating evidence showing that influenza infection and cardiorespiratory diseases are closely associated. Influenza has been described as a triggering factor capable of both exacerbate underlying chronic diseases as well as inducing the appearance of new respiratory and cardiovascular events. Consequently, influenza infection and its associated comorbidity have a significant impact on the health system. In this document, we extensively reviewed the current literature to describe the most relevant data on the relationship between influenza infection and cardiorespiratory diseases. Likewise, we analyzed the possible pathophysiological mechanisms explaining the connection between influenza infection and cardiac and respiratory events. Finally, reviewed data has been put into perspective to highlight the importance of influenza vaccination as an effective measure in the prevention of cardiorespiratory diseases, especially in the population with underlying chronic diseases.

Objective: To advance the development of an ideal and sustainable framework agreement for the public procurement of vaccines in Spain, and to agree on the desirable award criteria and their relative weight. Methods: A multidisciplinary committee of seven health-care professionals and managers developed a partial multi-criteria decision analysis to determine the award criteria that should be considered and their specific weights for the public procurement of routine vaccines and seasonal influenza vaccines, considering their legal viability. A re-test of the results was carried out. The current situation was analyzed through 118 tender specifications and compared to the ideal framework. Results: Price is the prevailing award criterion for the public procurement of both routine (weighting of 60% versus 40% for all other criteria) and influenza (36% versus 64%) vaccines. Ideally, 22 criteria should be considered for routine vaccines, grouped and weighted into five domains: efficacy (weighting of 29%), economic aspects (27%), vaccine characteristics (22%), presentation form and packaging (13%), and others (9%). Per criteria set, price was the most important criterion (22%), followed by effectiveness (9%), and composition/formulation (7%). Regarding influenza vaccines, 20 criteria were selected, grouped, and weighted: efficacy (29%), economic aspects (25%), vaccine characteristics (20%), presentation form and packaging (16%), and others (11%). Per criteria set, price was also the most relevant criterion (19%), followed by composition/formulation (8%), and effectiveness (8%). Conclusions: Contrary to the current approach, technical award criteria should prevail over economic criteria in an ideal and sustainable framework agreement for the public procurement of vaccines.

The childhood immunization schedule is well known and generally well implemented in developed countries. For various reasons, the same is not true of vaccines aimed at preventing infections in adults, in which vaccination coverage is incomplete and generally very deficient. In order to assess the situation of adult vaccination in Spain, the Fundación de Ciencias de la Salud has brought together a series of experts in different fields, including doctors, nurses, representatives of patient associations, health managers and economists, health authorities and journalists to deal with this issue. The format was that of a round table in which a series of questions previously formulated by the coordinators were to be answered and debated. The document presented is not an exhaustive review of the topic, nor is it intended to make recommendations, but only to give a multidisciplinary opinion on topics that could be particularly debatable or controversial. The paper reviews the main vaccine-preventable adult diseases, their clinical and economic impact, the possibilities of reducing them with vaccination programmes and the difficulties in carrying them out. The role of nursing, pharmacy services, patient associations and the health administration itself in changing the current situation was discussed. Prospects for new vaccines were discussed and we speculated on the future in this field. Finally, particularly relevant ethical aspects in decision-making regarding vaccination were discussed, which must be faced by both individuals and states. We have tried to summarize, at the end of the presentation of each question, the environment of opinion that was agreed with all the members of the table.

Invasive pneumococcal disease (IPD) and pneumococcal pneumonia (PP) represent an important health problem among aging adults and those with certain underlying pathologies and some diseases, especially immunosuppressed and some immunocompetent subjects, who are more susceptible to infections and present greater severity and worse evolution. Among the strategies to prevent IPD and PP, vaccination has its place, although vaccination coverage in this group is lower than desirable. Nowadays, there are 2 vaccines available for adults. Polysacharide vaccine (PPV23), used in patients aged 2 and older since decades ago, includes a greater number of serotypes (23), but it does not generate immune memory, antibody levels decrease with time, causes an immune tolerance phenomenon, and have no effect on nasopharyngeal colonization. PCV13 can be used from children 6 weeks of age to elderly and generates an immune response more powerful than PPV23 against most of the 13 serotypes included in it. In the year 2013 the 16 most directly related to groups of risk of presenting IPD publised a series of vaccine recommendations based on scientific evidence regarding anti-pneumococcal vaccination in adults with underlying pathologies and special conditions. A commitment was made about updating it if new scientific evidence became available. We present an exhaustive revised document focusing mainly in recommendation by age in which some more Scientific Societies have been involved.

En el año 2013 algunas de las Sociedades Científicas más directamente relacionadas con los grupos de riesgo para padecer enfermedad neumocócica publicamos un documento de Consenso con una serie de recomendaciones basadas en las evidencias científicas respecto a la vacunación antineumocócica en el adulto con condiciones especiales y patología de base. Se estableció un compromiso de discusión y actualización ante la aparición de nuevas evidencias. Fruto de este trabajo de revisión, en abril de 2017 se ha publicado una actualización del anterior documento junto a 4 nuevas Sociedades Científicas donde destaca, entre otras novedades, la recomendación de vacunación por criterio de edad. Se resumen algunas de las principales novedades que presenta la actualización del documento de Consenso

Streptococcus equi subsp. zooepidemicus infections are infrequent in humans. A clinical and epidemiological study of a milk-borne outbreak caused by this organism is described. Fifteen patients (5 females, 10 males) with a median age of 70 years (range 47-86) were infected. Twelve (80%) had underlying diseases. Infection with S. equi subsp. zooepidemicus presented as primary bacteremia in six cases, as bacteremia associated with aortic aneurism in four cases, as septic arthritis in two cases, as pneumonia in two cases, and as meningitis in one case. Five (33.3%) patients died. A case-control study proved that consumption of inadequately pasteurized cheese of a specific brand was associated with S. equi subsp. zooepidemicus disease (OR=4.5; 95% CI 1.57-19.27; p<0.001). This outbreak serves as a reminder that S. equi subsp. zooepidemicus causes serious infections that are usually zoonoses. Identification of beta-hemolytic streptococci to the species level to detect contaminated foods of animal origin is important for preventing new food-borne outbreaks. For a precise characterization of the isolates, the application of molecular markers is recommended.

Introducción. Los requisitos básicos para conseguir que una vacuna disminuya la incidencia y mortalidad de una enfermedad en la población son que sea eficaz, segura y estable, y que el país disponga de políticas vacunales que permitan conseguir y mantener coberturas vacunales altas, en la población diana. Pacientes y métodos. En general, la vigilancia de la varicela se limita a la declaración semanal numérica de los casos sospechosos. Esto dificulta caracterizar su patrón epidemiológico. Sin embargo, existen numerosos estudios, algunos de base hospitalaria, que describen y definen complicaciones, que junto a diferentes encuestas de seroprevalencia, han permitido avanzar en su conocimiento. Resultados. Varios de estos trabajos señalan que la percepción existente de que esta enfermedad es un proceso banal, merece algunas consideraciones. Por otro lado, la padecen casi el 100 % de las personas susceptibles, y la incidencia anual en una comunidad equivale prácticamente a una cohorte entera. Los resultados obtenidos en el "Estudio seroepidemiológico: Situación de las enfermedades vacunables en España", del Instituto de Salud Carlos III, subrayan que la infección se adquiere principalmente por debajo de los 6 años de edad, mientras que de los 6 a los 9 años es susceptible el 20,7 % de los niños/as, el 9,1 % de los de 10 a 14, el 4,9 % de los de 15 a 19 años, y el 2,6 % los de 30 a 39 lo que verifica un patrón típico de la áreas templadas. Conclusiones. La disponibilidad de vacunas eficaces definirán como la estrategia vacunal más adecuada para modificar el patrón epidemiológico de esta enfermedad la vacunación sistemática a los niños/as menores de 2 años de edad. Por otro lado, para reducir la bolsa de susceptibles en la población adolescente y adulta joven, y contribuir a prevenir el desplazamiento de la presentación de la enfermedad a la edad adulta, parecería conveniente vacunar sistemáticamente a los niños/as de 11 años, hasta que la cohorte de primovacunados menores de 2 años cumpla esa edad (AU)

BACKGROUND: A) To ascertain the rate of carriers and the Types of Neisseria Meningitidis circulating in the population resident in the health jurisdiction of Gran Canaria. B) to ascertain the pattern of distribution of such carriers. METHODS: A descriptive transversal design was made, with a random sampling in multiple stages and by conglomerates. A minimum sample size was determined at 707 individuals for an expected prevalence of 8.6%, with a rate of reliability of 95.6% and a precision of 0.02. Assuming that 15% if the individuals would not be willing to co-operate, the sample size was increased to 831 individuals, distributed in each conglomerate in proportion to the existing population. This size was distributed in turn into four groups by age and sex, in proportion to their significance in each basic health care zone selected at random. The individuals in the sample were identified from among those who attended the blood extraction units, and after they had passed the criteria of exclusion, their co-operation was requested as volunteers in the study. If they accepted, a questionnaire was filled out with a number of variables of epidemiological interest and a pharyngeal smear was taken. Since the Primary Care units were selected on a simple random basis, and the same method was used to select the individuals within the units, the estimate of the prevalence was made by means of an unbiased estimator. RESULTS: A total of 828 samples were obtained, that is, 99.6% of the number foreseen. With the exception of three, all of the individuals selected participated voluntarily in the study, a circumstance rendering it highly representative. All of the strains obtained corresponded to N. Meningitidis Serogroup B, except for one identified as N. Meningitidis Serogroup C Sero/Subtype 4:P1.2,5. The strains of N. Meningitidis serogroup B identified corresponded to 25 different sero-subtypes. The prevalence determined after having studied the sample was 6.45%, the variance = 0.0275 and the standard error = 1.66. We can conclude with a 95% degree of reliability that the prevalence of carriers of N. Meningitidis in the Gran Canaria health jurisdiction is estimated to be between 3.2% and 9.7%. CONCLUSIONS: A clear predominance of N. Meningitidis serogroup B strains among carriers has been verified. There are no statistically significant differences in the prevalence observed among the different age groups nor between sexes.
